The Michigan Department of Health and Human Services, on behalf of the Governor’s Nursing Home Workforce Stabilization Council, is seeking front-line nursing home staff to provide input on issues related to working in a nursing home. This includes the direct care workforce (CNAs, Aides, LPNs, RNs), nutrition services, custodial and housekeeping, and other frontline workers. Your answers are entirely anonymous and will be reported in combination with other respondent data. While we do gather general demographic information (e.g., length of time working in industry, age, race, etc.), we do not ask for any specifics such as your name or employer information. As such, your responses cannot be tracked back to you nor used in any way that could harm your employment. The Council is looking for open and honest feedback.

The survey is available in English, Arabic, and Spanish. There is a language selection drop-down box in the top right corner of the survey. The survey is a maximum of 11 questions and should only take a couple of minutes to complete. Please complete the survey by September 22nd.
